For PRE-ETS, develops work try-outs, situational assessments, internships and summer employment for students with disabilities working with a support team to assess a student's vocational strengths and barriers.
Essential Functions:
1.
Maintains communication with the Placement Supervisor and Program Manager regarding individual needs and job site availability 2.
Uses creativity in developing jobs within the community for individuals and students with a disability, including work try-out for students, situational assessments, and community or school internships.
3.
Meets with Placement Team (Program Manager, Placement Supervisor, ACCES-VR, Vocational Rehabilitation Counselor, MSC, and Collateral) to coordinate placement match, assess and discuss supportive services needed, establish goal and to update progress.
4.
Conducts on-site training, supervision, work performance assessments, and any other supported services as needed.
Willing to work side by side with the individual and educates employer on the rewards of hiring individuals with a disability.
(tax incentives, free on the job training etc.
) 5.
Coordinates transportation needs and availability for individual to travel to and from work 6.
Assists individuals and students with career exploration through means of online evaluations, internships, 7.
Provide job coaching for individuals for successful employment.
Also provide job coaching for in school job sites such as the cafeteria, library, with custodial staff, if needed.
8.
Reports and records designated monthly progress to ACCES-VR and OPWDD.
Maintains electronic records and files, evaluates and modifies performance and standards on a daily and weekly basis.
9.
Develops and conducts regular assessment for feedback and data on individual satisfaction and goal progress.
10.
Drives Agency vehicle as needed.
11.
Interrupts and reports suspected abuse to administrator immediately.
12.
Employee conducts themselves in conformity with the HIPAA Compliance Program, applicable institutional policies and procedures for individual privacy and the Maryhaven Electronic Information Security Policies and Procedures.
13.
Job requires access to all individual protected health information (PHI), in accordance with the Health Information Privacy and Accountability Act (HIPAA) regulations.
14.
Keeps immediate supervisor apprised of all significant issues.
15.
Follows agency policy in regards to attendance.
16.
Follows agency policy in regards to lateness.
Education and
Qualifications:
1.
BA degree and enrollment in a Master's degree program in vocational rehabilitation or related area; or BA degree and two years' experience providing vocational assessment services.
2.
Possess and maintain a valid NYS driver's license within Agency criteria and own a vehicle.
